  • Death, Disability or Retrenchment

    What do I do when a policyholder dies?

    • Complete the claimant declaration form
    • Attach the supporting documents: death certificate, cause of death and the affidavit if already available
    • The succession must sign the consent form if an inquiry is requested


    What do I do when I or the policyholder becomes disabled?

    • Complete the claimant declaration form
    • Attach the supporting medical reports
    • Sign the consent form authorising us to ask the medical attendant to complete the total and permanent disability questionnaire


    What do I do if I am retrenched? 

    • Send us an email
    • Attach the letter received from the human resources officer of the company, giving reasons for retrenchment
  • Pensions

    What do I do if I want my pension benefits to be credited to another bank?

    Send us an email with a copy of the bank account evidence.
